Brief Paperwork
Upon
entering the office, we'll have some brief paperwork for you to fill out.
This provides us with general information about you and your
condition.
Consultation Once the paperwork is completed, you will have a consultation with the
doctor to discuss your specific health-related problems and concerns, as well as, potential treatment options.
History
and Examination
Your symptoms may just be the 'tip of the iceberg.' In
order to determine the actual cause of your problem, the doctor will ask you
various questions related to your condition.
Next, he will conduct a thorough
orthopedic, neurological, and chiropractic examination to determine the nature of your condition, and appropriate course of treatment.
X-ray, MRI, CAT Scan & Lab Studies
If your
condition requires further diagnostic tests, we will make every effort to refer you to the appropriate source.
Same Day Treatment
Once
the examination and all appropriate studies are completed, patients
will generally receive their first treatment during the same visit.
This may include spinal adjustments, physiotheraperies and/or acupuncture.
Home Instructions
You will be given instructions on certain activities
or procedures to be conducted at home. These may include a combination of ice or heat application instructions, avoidance of certain
activities/positions, and exercises/ stretches.
Schedule Your Next Appointment
You will be given a convenient appointment time for your follow-up visit. A typical treatment plan would start with treatment
2-3 times per week for a few weeks, or until progress is made, then on a reduced frequency basis.
Office Policies:
We at the
Chiropractic Health Center understand that going to a new doctor's
office can be a nerve-racking experience. We aim to remove
unnecessary stress by providing you with the information you need to
make informed decisions based on realistic expectations.
We participate with many insurance companies including Aetna, Anthem, Blue Cross/ Blue Shield, Cigna, Connecticare, Healthnet, Landmark, Medicare and many others. We also accept Workers Compensation, auto accident and Personal Injury cases.
We verify insurance benefits as a
courtesy to the patient; however, insurance coverage is not a guarantee of
payment.
Few insurances cover acupuncture, but it may be covered under a flexible spending or health savings account. Our policy is to collect payment at the time of service and provide you with a receipt that you may submit for reimbursement.
Payment is expected on the same day as service.
We accept cash, personal checks, Visa & Mastercard.
Confidentiality: All patient information is held in strict confidence as required by law.
Cancellation Policy:
We are blessed to have a busy practice. When you make an appointment, that time is reserved for you. If a
cancellation is absolutely necessary, the courtesy of a 24-hour notice is required so others in need may be treated. We value and respect our patients and deserve the same in return.
